Thank you for your interest in participating with the Canadian Anesthesia Research Foundation Research for Research program. By volunteering your time to complete market research questionnaires you will be supporting CARF’s ability to generate much needed funding to further Canadian anesthesia research initiatives.

How the Program Works

From time to time you will receive a questionnaire distributed by CARF on behalf of a company wishing to investigate an area in anesthesia. The questionnaires will be pre-approved by CARF and only distributed to those anesthesiologists who have agreed to participate in the program. For each completed questionnaire returned to CARF the Company sponsoring the program will make a donation to CARF. The size of the donation will depend on the length of the questionnaire and the minimum donation will be $10.00 per returned, completed questionnaire. As a participant you can opt out of the program at any time.

Confidentiality and Personal Information

Your name, email address and any other identifying information will be used only by CARF for the purposes described in this document. Your information will be secured in a password protected system and only the administrator of this program will have access to this information. The Administrator of the system is under a confidentiality agreement with CARF to safeguard any personal information collected. The information you provide when filling out a questionnaire will be supplied to the sponsor company without any information that would identify you personally. Should you choose to opt out of the program your personal information will be deleted from the system with the exception that the Administrator will keep a note that you do not wish to participate.
